The Home and Community Care Program, is commited to provide holistic care, when caring for the First Nation community members who require assistance with their physical, emotional, mental or social needs in order to live independently in the community.
As part of our holistic approach, we want to support you when having difficult conversations with family members by offering you an electronic copy of “Departing Details”. We want you to have the necessary tools to assist you in organizing your wishes for the last stages of life.
